Normal Common Bile Duct Diameter on Cholangiography
The common bile duct (CBD) is the principal conduit for bile transport from the liver and gallbladder to the duodenum. Accurate measurement of CBD diameter is critical in pediatric patients, where biliary dilatation may indicate obstruction, choledochal cyst, or other hepatobiliary pathology. Age-appropriate reference values are essential, as the duct grows proportionally with the child.

Clinical Significance
CBD diameter increases with age throughout childhood, and exceeding the age-specific upper limit of normal should prompt further evaluation. Even modest dilatation in a neonate (greater than 2 mm) warrants consideration of biliary atresia, choledochal malformation, or neonatal hepatitis, as the consequences of delayed diagnosis are severe.
In older children, a dilated CBD may reflect choledocholithiasis, extrinsic compression, primary sclerosing cholangitis, or a choledochal cyst (Todani classification). It is important to note that post-cholecystectomy patients and those with a history of biliary instrumentation may have a physiologically larger duct, even in the pediatric population.
- Choledochal cyst (Todani type I–V) — most common cause of significant pediatric CBD dilatation
- Biliary atresia — typically presents with a diminutive or absent CBD in neonates
- Choledocholithiasis — uncommon in children; consider hemolytic disorders
- Primary sclerosing cholangitis — may show beading rather than uniform dilatation
- Extrinsic compression — pancreatic or lymph node pathology

Imaging Notes
On cholangiography — whether intraoperative, percutaneous transhepatic, or endoscopic retrograde (ERCP) — the CBD maximal diameter should be measured at its widest point in a plane perpendicular to the duct lumen, excluding the wall. Fluoroscopic cholangiography benefits from adequate contrast opacification and correct patient positioning to avoid foreshortening artifacts that may underestimate true diameter.
When interpreting pediatric cholangiograms, magnification factors must be accounted for if measurements are taken directly from film or digital images without calibration. Correlation with ultrasound and MRCP is recommended when cholangiographic findings are equivocal, as these modalities provide complementary cross-sectional anatomical detail without ionizing radiation.
