Normal Gallbladder Wall Thickness on Ultrasound

The gallbladder wall is a thin, layered structure best evaluated with ultrasound during fasting, when the gallbladder is distended. Accurate measurement of wall thickness is essential because focal or diffuse thickening serves as a key imaging marker for a wide range of biliary and systemic diseases.

Clinical Significance

In fasting adults, a wall thickness of 3 mm or greater is considered abnormal and warrants clinical correlation. Thickening may be focal or diffuse and can arise from primary gallbladder pathology or secondary systemic causes, making careful clinical integration essential.

In pediatric patients, the upper limit of normal is age-dependent, ranging from 2 mm in children aged 2–5 years to 3 mm in infants and older children through age 16. Measurements should always be obtained in a fasted state, as a non-distended gallbladder can produce spuriously increased wall thickness.

  • Acute cholecystitis — diffuse thickening often >4–5 mm with pericholecystic fluid and sonographic Murphy sign
  • Chronic cholecystitis — uniform thickening with a contracted gallbladder
  • Gallbladder adenomyomatosis — focal or segmental thickening with intramural Rokitansky-Aschoff sinuses
  • Systemic causes — hypoalbuminemia, heart failure, hepatitis, and cirrhosis can all produce diffuse secondary thickening
  • Gallbladder carcinoma — irregular asymmetric thickening or mass, particularly in the fundus

Imaging Notes

Gallbladder wall thickness should be measured on ultrasound after a minimum 4–6 hour fast in adults (shorter fasting periods are acceptable in young children). The measurement is taken at the anterior wall in the longest axis view, placing calipers from the echogenic mucosal surface to the outer serosal margin. The anterior wall is preferred because posterior acoustic enhancement can artificially obscure the posterior wall margins.

Key technique pearls include ensuring adequate gallbladder distension before concluding that thickening is pathologic, using high-frequency linear or curved transducers for optimal resolution, and documenting the presence or absence of associated findings such as pericholecystic fluid, gallstones, or intramural echogenic foci (Rokitansky-Aschoff sinuses) that help narrow the differential.
