Normal Portal Vein Diameter on Ultrasound
The portal vein is the principal vessel delivering nutrient-rich blood from the gastrointestinal tract and spleen to the liver, formed by the confluence of the superior mesenteric and splenic veins. Accurate measurement of portal vein diameter on ultrasound is a key component of hepatic and vascular assessment. Diameter measurement serves as a reliable, non-invasive marker for detecting portal hypertension and guiding further clinical workup.

Clinical Significance
In adults, a portal vein diameter exceeding 13 mm is considered abnormal and is the primary ultrasound threshold for suspecting portal hypertension. Dilation reflects elevated portal pressure, most commonly due to chronic liver disease and cirrhosis. Diameter alone, however, should always be interpreted alongside other sonographic findings such as splenomegaly, ascites, and reversal of portal venous flow.
In the pediatric population, expected diameters increase progressively with age, from approximately 5 mm in infants under 1 year to 10 mm in adolescents aged 11–16 years. Measurements exceeding age-adjusted norms should prompt evaluation for portal hypertension secondary to biliary atresia, congenital hepatic fibrosis, or portal vein thrombosis. Pitfalls include patient hydration status, depth of inspiration, and transducer angle, all of which can artificially influence measured diameter.

Imaging Notes
On grayscale ultrasound, the main portal vein is best measured in an oblique subcostal or right intercostal approach, with the patient in a supine or left lateral decubitus position during quiet respiration. The inner-to-inner wall diameter should be obtained in a true axial cross-section of the vessel, perpendicular to its long axis, ideally at the level of the porta hepatis. Doppler interrogation should accompany diameter assessment to evaluate flow direction and waveform characteristics.
Consistent technique is critical: measurements taken during deep inspiration may transiently increase portal vein caliber and lead to overcalling dilation. Comparison with prior studies and correlation with clinical history, liver echogenicity, and spleen size will maximize diagnostic accuracy.
