Normal Hip Alpha Angle Size on Ultrasound

The alpha angle is a key sonographic measurement used to assess the bony acetabular coverage of the femoral head in infants and young children. It is measured on coronal ultrasound images of the hip and quantifies the depth and morphology of the acetabular roof. Accurate measurement is essential for the early diagnosis and management of developmental dysplasia of the hip (DDH).

Clinical Significance

A normal alpha angle measures >60° on coronal ultrasound, indicating adequate bony acetabular coverage. Values at or below this threshold suggest acetabular dysplasia and are used to grade hip morphology according to the Graf classification system. Infants with alpha angles between 50–59° (Graf IIa/IIb) may require close surveillance or bracing, while angles below 50° (Graf III–IV) often necessitate active intervention.

The alpha angle is particularly important in the neonatal period when the acetabulum is predominantly cartilaginous and therefore not visible on plain radiographs. Measurement errors can arise from improper probe positioning, oblique coronal planes, or non-standard image acquisition, leading to either over- or underdiagnosis of dysplasia.

  • Developmental dysplasia of the hip (DDH) — most common cause of a reduced alpha angle
  • Physiologic immaturity — transient low alpha angle in neonates, especially preterm infants

Imaging Notes

On ultrasound, the alpha angle is measured in the standard coronal plane of the hip, obtained with the infant in a lateral decubitus position. The probe is placed over the lateral hip to obtain a true coronal section demonstrating the iliac baseline, the bony acetabular rim, and the labrum simultaneously. The alpha angle is formed between the vertical line of the iliac baseline and the line drawn along the bony acetabular roof; a correctly acquired image shows a flat iliac line and a clearly defined bony rim.

Strict adherence to the standard image plane is critical — even slight probe angulation can artificially alter the alpha angle by several degrees. Both hips should be evaluated and compared. Ultrasound is the modality of choice in infants under approximately 4–6 months of age, before sufficient ossification of the femoral head epiphysis permits reliable plain radiograph interpretation.
