Normal Metacarpophalangeal Joint Space Size on Radiography
The metacarpophalangeal (MCP) joint space represents the cartilage-lined gap between the metacarpal head and the proximal phalangeal base, visible on plain radiographs as a radiolucent interval. Accurate measurement of this space is essential for early detection of inflammatory arthropathies, where cartilage loss produces characteristic joint space narrowing.

Clinical Significance
A normal MCP joint space of 1–2 mm on posteroanterior (PA) and lateral radiographs reflects intact articular cartilage. Values below this threshold — particularly when bilateral and symmetric — raise concern for cartilage destruction. Asymmetric narrowing may suggest post-traumatic or degenerative etiologies, while uniform narrowing across multiple MCPs is a hallmark of systemic inflammatory disease.
Joint space widening beyond 2 mm is less common but may occur with joint effusion, ligamentous laxity, or in certain crystal deposition disorders. Subchondral erosions, periarticular osteopenia, and soft-tissue swelling are important accompanying findings that help narrow the differential.
- Rheumatoid arthritis — symmetric narrowing, periarticular erosions, ulnar deviation
- Psoriatic arthritis — asymmetric involvement, pencil-in-cup deformity at IP joints
- Calcium pyrophosphate deposition (CPPD) — chondrocalcinosis, hook-like osteophytes at MCPs
- Osteoarthritis — primarily affects IP joints; MCP involvement suggests secondary OA
- Hemochromatosis — MCP narrowing with hook osteophytes, particularly at 2nd and 3rd MCPs

Imaging Notes
MCP joint space is best evaluated on a posteroanterior (PA) radiograph with the hand flat on the detector and the X-ray beam centered over the metacarpal heads. The lateral view provides supplemental assessment but is less reliable for joint space measurement due to overlapping structures. Proper positioning — fingers extended and parallel to the detector — is critical to avoid artifactual narrowing from obliquity or flexion.
When assessing for subtle narrowing, comparison with the contralateral hand or serial radiographs over time is more informative than a single study. Magnification views or fluoroscopic spot images can improve visualization in equivocal cases. Note that radiography detects narrowing only after significant cartilage loss has occurred; early synovitis may be radiographically occult.
