Why Use

Pearls / Pitfalls

Utilizes standard, easily accessible measures such as lipid levels, fasting glucose, and BMI to estimate insulin resistance. Validated against the euglycemic-hyperinsulinemic clamp (EHC), considered the gold standard for measuring insulin resistance. The original derivation study focused on a group of Mexican adults, while several validation studies have examined various Asian populations. Further research is needed in other ethnic groups. Different studies have utilized varying cutoff values. This iteration relies on the original derivation study, where a cutoff >50.39 indicated the greatest risk for developing T2DM. There may be other potential future uses for this tool, as it has also been studied to predict hypertension and cardiovascular disease in specific patient populations.

Advice

Individuals found to be at a high risk for developing T2DM may benefit from initiation of preventive strategies, such as changes in diet and other lifestyle modifications. This tool is not intended to diagnose T2DM. Clinicians should still integrate other standard clinical assessments and markers, such as HbA1c levels, into their diagnostic and screening process.
