Normal Acetabular Angle Size in Boys: X-Ray & CT

The acetabular angle (also called the acetabular index) is formed between the Hilgenreiner line and a line drawn along the acetabular roof on an anteroposterior pelvic radiograph. It serves as a key quantitative marker of acetabular development in infants and young children. Accurate measurement is essential for the early detection and monitoring of developmental dysplasia of the hip (DDH).

Clinical Significance

The acetabular angle decreases with normal skeletal maturation as the acetabulum deepens and the femoral head applies appropriate mechanical stimulus to the cartilaginous roof. In boys, values typically decline from approximately 22–31° at birth to 15–23° by age 2. An angle persistently above the upper limit of normal for age raises concern for inadequate acetabular coverage.

Values exceeding the age-specific upper threshold are the primary radiographic criterion supporting a diagnosis of DDH or residual acetabular dysplasia following treatment. Asymmetry between hips of more than 2° may also be clinically significant even when absolute values appear borderline.

  • Developmental dysplasia of the hip (DDH) — most common cause of an elevated acetabular angle in infancy
  • Residual dysplasia post-reduction — failure of the angle to normalize after closed or open reduction
  • Neuromuscular hip dysplasia — e.g., cerebral palsy, spina bifida
  • Skeletal dysplasias — achondroplasia and related conditions may alter acetabular morphology
  • Technical error — pelvic rotation or tilt artifactually alters measured angles; always assess symmetry of the obturator foramina

Imaging Notes

On anteroposterior radiography, the acetabular angle is measured by drawing the Hilgenreiner line (horizontal line through both triradiate cartilages) and a second line along the most lateral ossified margin of the acetabular roof. Patient positioning is critical: the pelvis must be neutral, with symmetric obturator foramina confirming no rotation. Even mild rotation introduces systematic measurement error. CT is occasionally used for three-dimensional acetabular assessment in older children or pre-surgical planning, but plain radiography remains the standard for serial monitoring in infancy. On CT, multiplanar reconstructions in the coronal plane can replicate the radiographic measurement when standard projections are unavailable.
