Why Use

Provides a non-invasive, evidence-based method to estimate diabetes risk. Helps prioritize patients for further diagnostic evaluation (e.g., HbA1c or fasting plasma glucose testing). Supports proactive diabetes prevention strategies in at-risk populations.

When to Use

Use during routine visits for adults without a prior diabetes diagnosis to identify those at increased risk of developing type 2 diabetes mellitus or prediabetes. May be clinician- or self-administered.

Pearls / Pitfalls

Does not replace diagnostic testing for diabetes; an elevated risk requires follow-up testing. Does not account for ethnicity; certain populations (e.g., African American, Native American, Asian American, Hispanic/Latino, Pacific Islander) have an elevated risk for developing type 2 diabetes. Found to have a sensitivity of 79% and specificity of 67% in the original study.

Management

Score ≥5: Patient is at high risk for type 2 diabetes. Recommend confirmatory testing (e.g., fasting glucose, HbA1c). Counsel on lifestyle modifications, such as diet and exercise, to reduce diabetes risk. Consider referral to diabetes prevention programs or nutrition specialists, if available. Score <5: Provide general advice on maintaining a healthy lifestyle. Reassess risk periodically. Keep in mind that the patient may still be at risk for type 2 diabetes.

This tool is intended for risk assessment and screening purposes. Patients with an elevated score or symptoms of diabetes should undergo formal diagnostic testing.
