Estimates functional capacity.

Why Use

Focuses on common activities of daily living, providing a functional score that directly relates to everyday capabilities.

When to Use

Useful for estimating a patient’s functional capacity during preoperative assessments for surgery. Assesses functional capacity before and during exercise programs, such as cardiac rehabilitation, to tailor and monitor progress.

Formula

Addition of the selected points (self-reported by patient): Is the patient able to: Yes No Take care of self (e.g. eating, dressing, bathing, using the toilet) +2.75 0 Walk indoors +1.75 0 Walk 1–2 blocks on level ground +2.75 0 Climb a flight of stairs or walk up a hill +5.5 0 Run a short distance +8.0 0 Do light work around the house (e.g. dusting, washing dishes) +2.7 0 Do moderate work around the house (e.g. vacuuming, sweeping floors, carrying in groceries) +3.5 0 Do heavy work around the house (e.g. scrubbing floors, lifting or moving heavy furniture) +8.0 0 Do yardwork (e.g. raking leaves, weeding, pushing a power mower) +4.5 0 Have sexual relations +5.25 0 Participate in moderate recreational activities (e.g. golf, bowling, dancing, doubles tennis, throwing a baseball or football) +6.0 0 Participate in strenuous sports (e.g. swimming, singles tennis, football, basketball, skiing) +7.5 0

Advice

Scores are correlated with VO 2 max and metabolic equivalents (METs), which are typically measured through cardiopulmonary exercise testing (CPET). Higher scores indicate better functional capacity. In preoperative assessments: A score ≥34 suggests a lower perioperative risk. A score <34 is associated with an increased perioperative risk. Functional capacity is an important part of preoperative cardiac risk assessment, but additional patient factors such as comorbidities, type of surgery, and symptoms should also be evaluated.

More Information

Interpretation: VO 2 peak (mL/kg) = 0.43 × DASI + 9.6 METs (metabolic equivalents) = VO 2 peak / 3.5 VO 2 peak is maximal oxygen consumption (gold standard measurement is by exercise stress testing).
