Normal Liver Diameter Size by Age: Ultrasound, CT & MRI
The liver is the largest solid abdominal organ, and accurate measurement of its craniocaudal diameter is a fundamental component of abdominal imaging evaluation. The midclavicular line (MCL) is the standard reference axis used to assess liver span, as it reproducibly captures the greatest hepatic dimension. Accurate size assessment is critical for diagnosing hepatomegaly, monitoring chronic liver disease, and evaluating pediatric development.

Clinical Significance
In adults, a midclavicular liver diameter exceeding 155 mm is generally considered hepatomegaly. In children, normal liver span increases progressively with age — from approximately 90 mm in infants aged 1–3 months to 145 mm in adolescents aged 15–17 years. Exceeding age-adjusted thresholds warrants further clinical and laboratory correlation.
Hepatomegaly is a nonspecific finding and can reflect a wide range of underlying pathology. Pitfalls include overestimation due to a Riedel’s lobe (a normal anatomical variant with tongue-like inferior right lobe extension) and underestimation in patients with cirrhosis, where lobar redistribution may reduce overall liver span despite significant disease.
- Fatty liver disease (NAFLD/NASH) — most common cause of diffuse hepatomegaly in adults
- Hepatic congestion — right heart failure, Budd-Chiari syndrome
- Infiltrative disease — amyloidosis, hemochromatosis, lymphoma
- Viral or autoimmune hepatitis — acute phase enlargement
- Storage disorders — glycogen storage disease, Gaucher disease (especially in children)

Imaging Notes
On ultrasound, liver diameter is measured in the midclavicular line using a longitudinal (sagittal) scanning plane, from the dome of the diaphragm to the inferior tip of the right lobe. This is the most widely validated technique and the basis for published reference ranges. Adequate patient breath-hold at end-inspiration optimizes visualization of the hepatic margins. On CT and MRI, craniocaudal liver span can be measured on coronal reformats or calculated from sequential axial slices; however, published normative data are primarily derived from sonographic studies, so values should be interpreted with the modality in mind. Consistent use of the MCL landmark across studies ensures comparability.
